Process Safety Management Implementation at a Mature Asset

This paper was first presented at the Hazards Australasia Conference, June 2018, paper ref: HA 18023 

Following the Piper Alpha disaster, significant improvements were made to formalise safety assessments and in turn, identifythe controls needed to prevent major incidents. This article describes the successes and difficulties of the critical control approach (CCA) including the shift in thinking required on the part of process safety experts as well as changes to organisational structure. This unconventional approach is discussed with the view of its effectiveness in making process safety management more implementable and sustainable.
